High school student Akira Fudo lives with his only friend and longtime crush Miki Makimura, her brother Taro and her parents Noel and Akiko because his parents, who are doctors, work traveling abroad. One day Akira attempts to defend Miki from a local gang led by rapper Wamu and is rescued by his childhood friend Ryo Asuka, a child prodigy and American university professor. Ryo tells Akira that his recent expedition to the Amazon rainforest revealed the existence of an ancient race of demons but that the world's governments are suppressing this. Seeking Akira's help to expose the demons' existence, Ryo takes Akira to a dance club called Sabbath. Akira is horrified when Ryo begins slashing people with a broken bottle; the violence causes several demons to appear, and they massacre the remaining humans while Ryo films it. Ryo cries out to a powerful demon named Amon, ordering him to devour Akira, but Akira's will overpowers Amon, transforming him into Devilman and allowing him to slaughter the demons. Akira's body has been physically altered by his demonic encounter, giving him strength and athleticism.
Ryo and Akira proceed to fight several demons in an attempt to stop the coming invasion of Hell. Among the demons he fights are Silene and Kaim, comrades of Amon who seek to bring him back. Silene, Amon's lover, attempts to seduce Akira in order to bring out Amon but fails, leading to a fatal battle that leads to both Silene and Kaim's deaths. Akira later comes into conflict with Jinmen, a massive, sociopathic turtle demon who absorbs the faces of his victims into his shell. After possessing Akira's father, Jinmen goes on sadistic killing spree and kills Akira's mother. Though Akira manages to kill Jinmen, the loss of his parents deeply devastate him. Though they manage to defeat several demons, Akira's and Ryo's friendship becomes strained over time due to their differing personalities and beliefs, nearly coming to blows with each other when Ryo begins killing anyone who may know Akira's secret, even attempting to murder Miki.
One day, Ryo plans to expose the demons' existence by revealing that famous track star Moyuru Koda is a demon. Ryo drugs Koda, forcing him to transform in public and kill most of the other runners. Against Ryo's advice, Akira helps Koda to escape, but the revelation of the existence of demons triggers a global panic. Koda takes refuge with Akira's help, and they are soon joined by Miki "Miko" Kuroda, another Devilman and a friend of Akira's from track. As the panic spreads, humanity begins turning on itself. Disgusted by these events and Ryo's apathy toward the chaos he has caused, Akira begins trying to find other Devilmen himself. Meanwhile, Akiko flees home after discovering that her son Taro is a demon. When Noel finds them, the demonically-possessed Taro is eating his mother's corpse. Unable to bring himself to put down Taro, Noel instead protects him when paranoid soldiers arrive to kill Taro. Though Akira attempts to protect them, they are ultimately gunned down by the military and Akira buries them outside the city.
Wanting to know the reasons behind his own actions, Ryo returns to the Amazon rainforest. After returning, he uses a worldwide broadcast to lie about the origin of demons, saying that people dissatisfied with society eventually evolve into demons. He demands that anyone acting even slightly differently should be killed, turning the worldwide panic into a full-fledged genocide. He also shows footage of Akira transforming into Devilman. When the broadcast ends, Akira is threatened by Wamu's gang, who have taken refuge at the Makimura residence. They relent when Miki reiterates her trust in him, but they still ask him to leave. While Wamu and fellow gang member Gabi choose to look after Miki with Miko, the other two gang members Hie and Babo betray them by leading a mob to the Makimura home. Wamu, Gabi, Miko and Miki are stabbed to death. Meanwhile, Akira finds himself protecting innocent people from being stoned by a mob. After he convinces the mob to stop, traitor Koda and several other demons appear and slaughter them all. Escaping from Koda, Akira returns to the Makimura residence. When he finds the mob parading in the street with his friends' dismembered body parts impaled on pikes, Akira denounces the mob as the real demons and incinerates them all.
In the end, Akira confronts Ryo, who reveals himself to be the fallen angel Satan, whom God exiled to Earth, where he discovered the demons. When God's angels descended on Earth and devastated it, the bodies of Satan and the demons were destroyed, but their souls endured their physical destruction. Having reincarnated as Ryo, Satan explains that his apocalypse is meant to exterminate humanity for defiling the world. Satan also reveals that he made Akira a Devilman to allow him to survive in the world to come, as his way of thanking Akira for being by his side when he was human. Akira refuses to join Satan's cause and gathers the other Devilmen to fight Satan's army. After killing Koda for his betrayal, Akira battles Satan but is ultimately defeated. With humanity finally obliterated and the Earth a devastated ruin, the victorious Satan reminisces with Akira until he realizes Akira has already died. Having killed the only person he ever loved, Satan becomes overwhelmed by the same emotions he previously denounced as a weakness and cradles Akira's lifeless body as the angels of God descend, destroying and reforming the Earth.
